Service Coordination
Services (a.k.a. Case Management) are designed, on an Individualized basis, to
assist our individuals with accessing supports within their communities in order
to afford a greater sense of Integration and Independence. The purpose of
Service Coordination Activities is to assure that each individual has the
opportunity to actively participate in planning their daily activities,
choosing their service providers and affecting their futures. Based on the
needs and desires of the individual, the Service Coordinator will assist the
individual in locating and securing a variety of services, including
educational, vocational, residential, financial, medical, and clinical
supports, as well as social and recreational opportunities.
Medicaid Service
Coordination (MSC) is the result of the consolidation of HCBS Waiver Service
Coordination and Comprehensive Medicaid Case Management systems. MSC took
effect on March 1st, 2000.
Eligibility for the MSC program
includes:
- Documented Developmental Disability
- Eligible for Medicaid
- Individual Does Not Live in ICF, Skilled Nursing Facility, Acute Care Hospital, or Psychiatric Center
- MSC Application Approval by Taconic DDSO
Medicaid Service Coordination (MSC) Activities include the following:
- Providing Information on Available Supports and Services
- Accessing, Linking and Referring Individual to Desired Services
- Monitoring Status in Various Services
- Intervening on Behalf of Individual and Assisting in Conflict Resolution
- Developing an Individualized Service Plan
- Reviewing Plan and Making any Necessary Changes
- Monitoring Individual's Satisfaction with Services
- Coordinating/Attending any Necessary Meetings
- Advocating on Behalf of Individual
- At Least One Face to Face Contact with Individual per Month
- Complete Service Coordination Agreement with Individual/Family
- Complete Service Coordination Activity Plan and Update as Needed
- Complete SCOR Report for Individuals Residing in Agency Sponsored, Certified Residences
- Complete ICF Level of Care Forms of Individual Participating on HCBS Waiver Program
- Attend 15 Hours of Approved Training per Year
- Act as Team
Leader for PARC Residential Individuals
PARC’s
Medicaid Service Coordination Unit currently provides services to 263
individuals of all ages, living in a variety of residential situations. The
Unit consists of a Director of Service Coordination, a Service Coordination
Supervisor, Senior Service Coordinator, 8 Full Time Service Coordinators, and
1 Part Time Service Coordinator, specializing in Traumatic Brain Injuries.
The Unit is overseen by PARC’s Associate Executive Director. Hours of operation
are Mon through Fri, 8:30 - 4:30, by appointment in the evenings and available
by emergency phone for emergencies.
